Scientific Diving Forms and Medical Evaluation
New scientific divers must complete all the forms listed below. You must have a diving medical evaluation performed by your physician who must authorize you for diving. SFSU Student Health Services may assist students with the divers medical exam, contact them directly. In addition to these forms, you will be required to provide copies of all your certification cards, dive logs for the past 12 months of dive activity and verification that scuba equipment (regulator, buoyancy compensator, dive cylinder, dive computer) has been professionally inspected in the past 12 months.
